The Periodic Health Assessment (PHA) is an annual requirement for all active duty service members during their birth month. PHA appointments are essential to assess numerous various aspects of care as they relate to a specific service members need. During the PHA Individual Medical Readiness (IMR) requirements and Deployment-Related Health Assessments are identified, preventative health education and occupational driven screenings are conducted, and medical referrals are placed as needed.
Electronic Periodic Health Assessment Process:
1. An online assessment must be completed. Access the online ePHA visiting the following link: https://eha.health.mil/EHA/MiddleZone/index.aspx?ALSErr=1
2. Once complete, notify the Medical Readiness team by sending an email to: usn.rota.navhosprotasp.list.medical-readiness-clinic@health.mil with the following information:
- Two (2) contact numbers (cellphone and work)
- Dates and time you are available to receive a phone call
- DOD ID Number.
NOTE: Do not send an email using the in-site email option. Your email must go to the address above.
3. Once screened by the Medical Readiness team, the member will receive a confirmation email and instructions to schedule an appointment with the provider and finalize your PHA. You may chose to schedule a virtual appointment (over the phone) or a face-to-face appointment.
4. Contact the Medical Readiness team for scheduling at:
DSN: 727-3185 or 727-3364 or 727-3625.
CELL: 956-82-3185 or 956-82-3364 or 956-82-3625.
5. At the conclusion of the appointment, the PHA will be complete, and the member’s readiness will reflect the completion date within the member’s IMR.
